HVPFP Requirements

Below is what we require from applicants for pet food assistance:

● Completed HVPFP Application;

● Copy of proof of financial need (most recent tax return, copy of annual earnings through SSI, welfare, disability, or other hardship);

● Copy of photo ID.

If you are unable to open the files for any reason, let us know and we will send the application to you by return e-mail or U.S. mail. 

Upon receipt of your completed application and required documentation, you will be called for a phone interview.  

Upon acceptance, a schedule will be set up for you to pick up your food monthly at the pantry.

We only provide assistance to owners who register and humanely care for their pets.
